Many believe a regular massage is a body rub with soothing music and a pleasant scent that invokes relaxation. Amy Montia, Ph.D., LMT, and a medical massage practitioner at Physio Logic, says a deep tissue session may include these elements but is not necessarily restorative.

The objective of a deep tissue massage is to facilitate the restrictions of joint knots, and adhesions in muscle and fascia, Montia explains. A massage that targets deep tissue is beneficial for someone willing to tolerate moderate pain.

Minehan usually starts on the back, as this is the most uncomfortable area. He then moves on to the legs, shoulders, arms, and finally the neck and head—which he finds most relaxing due to the many nerve endings in the scalp.

Montia starts with deep tissue classes, covering a broad area, and then focuses on specific muscles or areas. People have different pain thresholds, and deep tissue massage can cause discomfort or even mild pain. For this reason, Minehan says clients should control the depth and pressure of their massage.

While some discomfort is normal, it's not necessary. "If the method is performed too gently, the exercise cannot successfully dissolve the knots that form in the muscles" says Montia. However, too much stress or extreme discomfort can negate these benefits.

However, it is normal to feel soreness for a few days after a deep tissue massage. Deep muscle stripping and myofascial release (MFR) can cause bruising or pain, but the joint stretches performed during deep-tissue training don't cause pain for the next few days, Montia says.

Deep tissue massages vary in length, but are usually between 60 and 120 minutes. Minehan says 90 minutes is usually the ideal amount of time to fully resolve the primary ailment and to fully "touch" the rest of the body.

Swedish Massage & Deep Tissue Massage

Experts say there is overlap between Swedish massage and deep tissue massage techniques, but the biggest difference is the depth and perceived intensity of the pressure used.

"Swedish massage is more about promoting better blood flow to the muscles and fascia," says Montia. "Deep tissue classes focus more on the knots of the muscles and deep fascia. If you’re tense [or] sore from a pulled muscle, or you’ve got an old injury from a bad work environment, deep tissue training can be helpful."

According to Montia, massage guns can help eliminate knots and sore muscles, especially after strenuous exercise.
